Key Responsibilities - Community nurse:
- Conducting comprehensive assessments of patients' health needs and developing individualised care plans.
- Providing skilled nursing care, including wound care, medication administration, and health monitoring, in accordance with established protocols and guidelines.
- Collaborating closely with patients, families, and caregivers to educate them about managing health conditions and promoting healthy lifestyles.
- Administering treatments and interventions, including intravenous therapies and other specialised procedures, as required.
- Monitoring patients' progress and adapting care plans as needed to ensure optimal outcomes.
- Working closely with physicians, therapists, social workers, and other healthcare professionals to coordinate holistic care for patients.
- Responding to emergencies and crises in a calm and effective manner, ensuring patient safety and well-being.
- Documenting patient assessments, interventions, and outcomes accurately and timely in electronic health records.
- Ensuring compliance with infection control, health and safety regulations, and professional nursing standards.
- Providing emotional support and counselling to patients and their families, especially during challenging times.
Qualifications and Requirements:
- Registered Nurse (RGN) qualification and active registration with the Nursing and Midwifery Council (NMC).
- Previous experience in community nursing or a related field is preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Empathy and compassion for patients and their families.
- Ability to work autonomously and as part of a team.
- Proficiency in using electronic health record systems and basic computer skills.
- Willingness to travel within the community to provide care to patients.
- Valid driver's license (if applicable).
