Key Responsibilities - Community nurse:

1. Conducting comprehensive assessments of patients' health needs and developing individualised care plans.
2. Providing skilled nursing care, including wound care, medication administration, and health monitoring, in accordance with established protocols and guidelines.
3. Collaborating closely with patients, families, and caregivers to educate them about managing health conditions and promoting healthy lifestyles.
4. Administering treatments and interventions, including intravenous therapies and other specialised procedures, as required.
5. Monitoring patients' progress and adapting care plans as needed to ensure optimal outcomes.
6. Working closely with physicians, therapists, social workers, and other healthcare professionals to coordinate holistic care for patients.
7. Responding to emergencies and crises in a calm and effective manner, ensuring patient safety and well-being.
8. Documenting patient assessments, interventions, and outcomes accurately and timely in electronic health records.
9. Ensuring compliance with infection control, health and safety regulations, and professional nursing standards.
10. Providing emotional support and counselling to patients and their families, especially during challenging times.

Qualifications and Requirements:

1. Registered Nurse (RGN) qualification and active registration with the Nursing and Midwifery Council (NMC).
2. Previous experience in community nursing or a related field is preferred.
3. Strong clinical assessment and critical thinking skills.
4. Excellent communication and interpersonal abilities.
5. Empathy and compassion for patients and their families.
6. Ability to work autonomously and as part of a team.
7. Proficiency in using electronic health record systems and basic computer skills.
8. Willingness to travel within the community to provide care to patients.
9. Valid driver's license (if applicable).
